PathFinder Programs

 

Preventive Health Exam (Physical)

PathFinder provides on-site preventive health exams (physical). We believe this exam is the gateway to obtaining appropriate preventive health procedures and to identify and engage people with chronic disease risks into their management. Participants receive a thorough health history, assessment of chronic health risks, vital signs measurement, body metrics assessment and a comprehensive lab assessment. This data is compiled into a bound report which is reviewed with each participant at the time of their physical examination.

Appropriate referral to further healthcare resources are made as indicated and participants at risk for chronic disease are engaged in company's disease management offerings. Each participant also receives a personal prevention plan that outlines appropriate preventive health procedures required over the next 5-10 years.

 

Medically Monitored Weight Loss Program

Obesity is a modern day crisis and has become the number one preventive cause of death in the United States, overtaking smoking. The long term costs associated with the obesity epidemic are great. PathFinder believes that weight management should be a preventive health priority for any company. PathFinder offers an on-site medically monitored weight management program. This program focuses on long-term sustained change through a low calorie diet, exercise and ongoing education. Participants are assigned to a personal dietary counselor that meets with them face-to-face every 2 weeks for six months. A specific weight loss strategy with targeted calorie counts, specific dietary instruction and appropriate nutritional supplements is personalized for each participant. Important health indicators associated with weight loss are monitored, followed by our medical team and shared with each participant at the beginning and end of their program.

 

On-site Health Support Program

PathFinder offers an on-site health support program that focuses on two of the most important chronic conditions facing Americans today: diabetes and heart disease. Participants meet face-to-face with their coach 3-4 times a year to ensure they meet treatment goals, engage in health lifestyle changes, comply with medication and treatment recommendations and gain understanding of their health risks. Objective clinical data such as lab tests, vital signs and body metrics are followed and tracked with the participant as appropriate. All data and information is designed to be shared with their primary medical team.
