Our PathFinder mobile health centers provide on-site medical services for Preventive Health, chronic disease support, and acute care.

PathFinder Mobile Health Centers

Our PathFinders are designed to provide employees with a comfortable and positive health experience. They are fully functional medical centers with private exam rooms, nursing stations, lab centers, and waiting areas.

 

Preventive Health

PathFinder's program begins with Health Risk Assessments (HRA) and face-to-face counseling sessions designed to identify employees at risk for the most common chronic diseases and engage them to take control of their health care so they are treated to goal. The HRA includes a health risk questionnaire, a laboratory screening, and body metrics.

 

Chronic Disease Support (CDS)

PathFinder's CDS program focuses on diabetes, metabolic syndrome, high blood pressure, high cholesterol, asthma, tobacco use, and weight management. Employees enrolled in the program will receive a personalized program with one to four 25-minute support visits, depending on each individual's needs. Enrollees also receive additional blood pressure checks, weigh-in, and lab work between the support sessions.

During the support visits, PathFinder's medical staff educates your employees on their concerns and care options. Emphasis is placed on ensuring employees follow through on all recommend treatments so they meet established health goals. All results from this personalized approach can be integrated with the care participants receive from their personal physicians.

 

Acute care

Acute care can be provided for many of the common ailments affecting employees including colds, bronchitis, flu, stomach ailments, sore throat, sinusitis, minor injuries, strains and sprains, etc.

Visits can be scheduled the same day requested with minimal wait times. Common lab assessments and prescriptions are provided as necessary.
